TERMS AND CONDITIONS
I hereby represent that I am the legal owner of the dog described above as (the “Dog”) to be enrolled in PackLeader Academy classes, private sessions or Board & Train boot camp service.
I hereby release PackLeader Academy, its owners, representatives and agents from all liability which I, the dog, any third party or their pet may suffer, including specifically, but not without limitation, and injury or damage whatsoever arising from my actions or failure to act, the Dogs actions or generally the Dog’s attendance and participation at Packleader Academy private sessions, or their Website.
I hereby agree to indemnify and hold harmless Packleader Academy, its owners and agents from any and all claims, or claims by myself or third party accompanying me to a function of Packleader Academy, or while attending the premises thereof, as a result of any action or failure to act by me or the Dog.
I hereby represent that the Dog is of good health and has not been ill with any known contagious diseases within the last thirty days.
I recognize that the health of the Dog is my personal responsibility. I hereby represent that all required vaccinations for the Dog are up to date and will continue to remain so for as long as I use Packleader Academy services. I will also promptly provide proof of all booster vaccinations and stay up to date.
I further understand and agree that in joining the class, PackLeader Academy has relied on my representation that the Dog is in good health and has not harmed or shown aggression or threatening behaviour towards any person or any other dog.
I understand that by bringing the Dog to an open facility means that they will be interacting with other dogs. Minor cuts or scratches are inevitable, and, though the dogs are carefully supervised at all times, and the owners do their utmost to keep them safe, there is no warranty or representation by Packleader Academy that a more serious incident (scuffles, fighting, even death) will not occur. I accept the risk of any such serious incident.
I further understand and agree that Packleader Academy and their trainer will not be liable for any problems that might develop with the Dog, including, but not limited to, sickness, disease, injury, running away and death, provided that reasonable care and precautions are followed.
I understand I am solely responsible for any harm caused by the Dog to another dog or the trainer while they attend Packleader Academy. If for any reason, Packleader Academy is unable to continue classes or sessions for the Dog, I must remove the dog from class or cancel any further sessions with the dog.
I further understand and agree that any problem that develops with the dog will be treated as deemed best by the trainer of Packleader Academy at their sole discretion and that I assume full financial responsibility for any and all expenses involved. If the dog is removed from the class or the session is cancelled, we do not offer any refunds.
The hours of operation of Packleader Academy classes are 6:30 pm to 9:30 pm (“closing time”) Mondays. One-on-one private in-home sessions are Tuesday through Friday evenings and Saturdays. I agree that the management of, retains the right to amend the hours of operation at any time.
I understand that I must pay in full for services rendered before the beginning of class or session, and submit this signed form.
You must give us 48-hour notice if cancelling your classes or private session, or you will not qualify for a refund.
I understand that I must walk the Dog in and out of the facility on a leash, and in a safe and expedient manner. If I chose to go off leash, it is done at my own risk and responsibility.
I agree that the Dog may be videotaped, photographed, and /or recorded. Packleader Academy shall be the exclusive owner to the result and all proceeds of such taping, photography and recordings.
Packleader Academy reserves the right to permanently remove a Dog from class at any time.
If the Dog attends Packleader Academy Obedience classes on multiple dates, then this Agreement applies to each and every date, until the last day attending.
These above points apply to all Bed n’ Buddies board and train clients as well.
All the above points are included under this agreement. Failure to follow our policies can also result in loss of service and with no chance of a refund.
I, the Owner, certify on the date listed below, that I am eighteen years of age or older and have the legal capacity to enter into a binding contract. I am the only owner of the Dog, or if I am not, then I have advised PackLeader Academy of who the other owner(s) is (are). Further I certify that I have read and understand the terms and conditions set forth in this agreement. I agree to accept all the terms, conditions, and statements of this agreement.
